Reconstructive Surgery

Procedure

Although these three types of surgery share some common techniques and approaches, they have somewhat different emphases. Plastic surgery is usually performed to treat birth defects and to remove skin blemishes such as warts, acne scars, or birthmarks. Cosmetic surgery procedures are performed to make the patient look younger or enhance his or her appearance in other ways. Reconstructive surgery is used to reattach body parts severed in combat or accidents, to perform skin grafts after severe burns, or to reconstruct parts of the patient's body that were missing at birth or removed by surgery. Reconstructive surgery is the oldest form of plastic surgery, having developed out of the need to treat wounded soldiers in wartime.

BREAST

The most frequently performed cosmetic surgery is breast augmentation can give women with small or unevenly sized breasts a fuller, firmer, better-proportioned look through the placement of implants in the breast. Women may elect to undergo breast augmentation for many different medical and aesthetic motivations, including balancing breast size and compensating for reduced breast mass after pregnancy or surgery.

Mastopexy (Lift) A breast lift, or mastopexy, is performed to return youthful shape and lift to breasts that have sagged as a result of weight loss, pregnancy, loss of the skin's natural elasticity or simply the effects of gravity. The procedure can also reduce areolar size (the dark skin surrounding the nipple), and it can be combined with breast augmentation for added volume and firmness. Breasts of any size can be lifted, but results last longest when they are originally small and sagging.

Reduction :

Large breasts may cause physical and mental discomfort and can even harm the women who have them -- the size and weight of large breasts can result in self-consciousness, improper posture, pain in the back and neck, indentations from bra straps, skin rashes, breathing problems and skeletal deformities. Breast reduction surgery is usually done to provide physical relief from these symptoms.

Reconstruction :

Breast Reconstruction is commonly begun and sometimes completed immediately following mastectomy, so that the patient wakes with a new breast mound. Alternatively, reconstruction may begin years after mastectomy. There are several ways to reconstruct the breast, both with and without implants; your surgeon(s) should work together with you in deciding which is the best for you. Breast reconstruction has not been proven to affect the recurrence of cancer or other diseases, chemotherapy or radiation treatment.

BODY

Abdominoplasty :

Abdominoplasty is a surgical procedure that helps to flatten and shape the abdomen and waist by removing excess fat and skin and tightening muscles. The best candidates are in good physical condition with pockets of fat or loose skin that haven't responded well to diet and exercise. The procedure can also be useful for women with stretched skin and muscles from pregnancy.

The procedure takes approximately two to five hours and can be performed with general anesthesia or local anesthesia with a sedative. Recovery time ranges from two weeks to a few months. With a balanced diet and regular exercise, results are long-lasting. Abdominoplasty will likely leave a permanent scar stretching between the hips, although it should be low enough for even a bikini to conceal it.

Body Lift :

During the procedure, skin is lifted from underlying tissue, muscles are tightened and stitched and fat is removed where necessary, and the skin is sutured closed. The navel is often removed and replaced in its new position. Surgical tubes may be needed for a few days to drain any fluid, and patients may need to wear compression garments for a few weeks to faciliate faster healing.

After surgery, patients are encouraged to walk as soon as they are able, although stenuous activities must be limited for about a month. Results are visible immediately.

Liposuction :

One of the most popular cosmetic surgery procedures, liposuction sculpts the body by removing undesirable fat from areas such as the abdomen, waist, hips, buttocks, thighs, knees, chin, cheeks, neck and upper arms that have not responded to diet and exercise. Local anesthesia or general anesthesia may be used and the procedure can last anywhere from one to five hours depending on the amount of fat being removed.

Brachioplasty :

After substantial weight loss, weight gain or simply from the effects of aging, skin on the upper arms can become inelastic, loose and flabby. Arm lifts, also known as brachioplasty, remove that excess skin and fat. Patients who have significant amounts of fat may want to consider combining the procedure with liposuction

Benefits

Reconstructive surgery is often performed on cancer patients as well as on burn and accident victims. It may involve the rebuilding of severely fractured bones, as well as skin grafting. Reconstructive surgery includes such procedures as the reattachment of an amputated finger or toe, or implanting a prosthesis. Prostheses are artificial structures and materials that are used to replace missing limbs or teeth, or arthritic hip and knee joints.

In cancer patients, reconstructive surgery is done to restore the function as well as the appearance of the face and other parts of the body. The most commonly performed reconstructive surgeries of cancer patients are breast reconstruction, laceration repair, scar revision, and tumor removal.

The most challenging area of reconstructive surgery involves the structures of the face, neck, and jaw because trauma or cancer treatments often affect the patient's ability to see, eat, taste, swallow, speak, and hear as well as his or her external appearance. The surgeon must try to retain as much sensation as possible when performing skin or bone grafts in the head and neck as well as recreate a reasonably normal appearance.

Risk

The risks associated with plastic, cosmetic, and reconstructive surgery include the postoperative complications that can occur with any surgical operation under anesthesia. These complications include wound infection, internal bleeding, pneumonia, and reactions to the anesthesia.

In addition to these general risks, plastic, cosmetic, and reconstructive surgery carries specific risks:

  • formation of undesirable scar tissue
  • persistent pain, redness, or swelling in the area of the surgery
  • infection inside the body related to inserting prosthesis. These infections can result from contamination at the time of surgery or from bacteria migrating into the area around the prosthesis at a later time
  • anemia or fat embolisms from liposuction
  • rejection of skin grafts or tissue transplants
  • loss of normal feeling or function in the area of the operation (for example, it is not unusual for women who have had mammoplasties to lose sensation in their nipples)
  • complications resulting from unforeseen technological problems (the best-known example of this problem was the discovery in the mid-1990s that breast implants made with silicone gel could leak into the patient's body).

Medical

Some patients should not have plastic surgery because of certain medical risks. These groups include:

  • patients recovering from a heart attack, severe infection (for example, pneumonia), or other serious illness
  • patients with infectious hepatitis or HIV infection
  • cancer patients whose cancer might spread (metastasize)
  • patients who are extremely overweight. Patients who are more than 30% overweight should not have liposuction
  • patients with blood clotting disorders

Psychological

Plastic, cosmetic, and reconstructive surgeries have an important psychological dimension because of the high value placed on outward appearance in Western society. Many people who are born with visible deformities or disfigured by accidents later in life develop emotional problems related to social rejection. Other people work in fields such as acting, modeling, media journalism, and even politics, where their employment depends on how they look. Some people have unrealistic expectations of cosmetic surgery and think that it will solve all their life problems. It is important for anyone considering nonemergency plastic or cosmetic surgery to be realistic about its results. One type of psychiatric disorder, called body dysmorphic disorder, is characterized by an excessive preoccupation with imaginary or minor flaws in appearance. Patients with this disorder frequently seek unnecessary plastic surgery.
